Contracts to New Hampshire health clinics in eligible areas for screening services to detect female reproductive cancers. Funding is intended for programs providing services to underserved women ages 21 and above. Proposed programs must serve the women of Rockingham and Hillsborough Counties.

The NBCCEDP target population for the free screening program is uninsured or underinsured women who are at or below 250% of the federal poverty level, aged 40 to 64 years for breast cancer services, and aged 21-64 years for cervical cancer services. The NBCCEDP target population for the screening rate improvement program is all women between 21-75 years of age who have not been screened according to United States Preventive Services Tasks Force (USPSTF) guidelines.

The Department seeks to increase breast and cervical cancer screening rates among disproportionately affected populations by:

  1. Enhancing clinical service delivery among the appropriate patient population through implementation of evidence-based interventions, and
  2. Using a Community Health Worker (CHW) to increase screening rates among populations that have lower screening rates than the state average (e.g., low-income, less than a High School Diploma, etc.).

Estimated Size of Grant:

The Department is anticipating up to $40,000 available funds for each Contract Period.

Term of Contract:

Contract(s) will be effective upon Governor & Executive Council approval, through June 30, 2020 for Year 1 funding, from July 1, 2020 through June 30, 2021 for Year 2 funding, and from July 1, 2021 through June 30, 2022 for Year 3 funding.

The Department reserves the right to renew the contract(s) for up to two (2) additional years, subject to the continued availability of funds, satisfactory performance of services and approval by the Governor and Executive Council.

Additional Eligibility Criteria:

The Department anticipates awarding contract(s) to one or more vendors who meet the following criteria:
- Is a current Health System or clinic with the capacity and ability to provide these services or be able to provide documentation with Proposal that demonstrates support from a partner Health System.
- Can provide outreach and educational services focused on improving cancer screening rates of a specific population.
- Can develop a health system evidence-based intervention (EBI) implementation plan to improve cancer screening rates.
- Can provide services focused on assessing and addressing barriers to accessing cancer screening and follow-up diagnostics.
- Can obtain screening and, if applicable, diagnostic and treatment data and enter it into BCCP’s web-based data collection system – Med-IT.
- Has clinical staff, which may include: RNs APRNs, and/or MDs, available to assist and advise a Community Health Worker (CHW) on follow-up of any clients who require case management for diagnostics and/or treatment services.
